Support that fits everyday life
Support that starts with what matters to you.
Changes in routine can make familiar tasks feel less straightforward. You may be returning home after a hospital stay, finding it harder to organise the day or simply wanting someone to check in. Practical support can focus on the ordinary things that help you feel more settled, without taking away your choices.
A wellbeing visit provides a friendly opportunity to talk about how the day is going and identify everyday tasks you would like help with. These visits are planned non-medical support. They are not health assessments, clinical monitoring or an emergency response service. We will discuss expectations clearly when considering an arrangement.
After hospital discharge, support might include organising groceries, preparing a light meal or helping with household tasks. The service needs to be suitable for your circumstances and work alongside any care or clinical arrangements already in place. It does not replace a discharge plan, professional treatment or personal care.
Yasmin's background includes over 18 years in health and social care and more than eight years working as an Occupational Therapist within the NHS. That experience informs a respectful understanding of independence and wellbeing. Cheshire Home Support itself provides non-medical everyday support, rather than occupational therapy assessment or treatment.
